Dear Parent:

A Health History Form is required by Maryland State law to be on file for each of our KidShip students. You may download this form on our website www.kidshipsailing.com or call our office at (800) 639-9192. Please note that the law requires a new form for each student each year. If your child attended KidShip last year, a new form will still be required.

This form must be completed IN FULL (do not leave any blanks). Please bring the form to registration on the first day of class. THE HEALTH HISTORY FORM AND IMMUNIZATION RECORD MUST BE PRESENTED AT REGISTRATION ON THE FIRST DAY OF CLASS IN ORDER FOR YOUR CHILD TO ATTEND CLASS ON THAT DAY. NO EXCEPTIONS WILL BE MADE.

Please note that, by law, students are not allowed to carry any medications. Any medications must be turned in to and administered by the KidShip staff. All medications must be accompanied by a side-effect brief (available at pharmacy) to be kept on file at KidShip.

NOTICE: Every child attending KidShip must be signed in and signed out by a parent or other adult every day. If the accompanying adult is other than a parent or guardian of the child, he or she must have a note of authorization from the parent.
